Summary

This position is responsible for the overall operation of clinical care management functions through oversight of both daily and weekly team activities. Drives clinical and throughput integration, develops systems and processes for care management activities related to discharge planning. Oversees the collection, analysis, and reporting of financial data related to utilization management, process and performance improvement. Accountable for achieving established performance targets through actively engaging multiple disciplines and professional staff.

Appropriately manages people, relationships, and processes in order to achieve maximum results. Engages internal and external stakeholders in care management processes, promotes interdisciplinary collaboration, fosters teamwork, and champions service excellence in keeping with organizational goals. Position regularly partners with external stakeholders including payers, post-acute facilities, and home care service agencies to align with organizational goals. Possesses the ability to manage multiple tasks in a fast-paced environment.

Minimum Qualifications

  1. Must possess a current license to practice professional nursing in the Commonwealth of Pennsylvania.
  2. Master’s Degree in health-related field or business (e.g., MSW, MBA, MSN, MHA).
  3. Five years of experience in acute care management, utilization, or quality improvement
  4. Prior management experience.
  5. Basic computer skills in Microsoft Word, Excel and PowerPoint.
  6. Knowledge of state, federal and accrediting agency standards.
  7. Knowledge of care management and referral software system.
  8. Ability to gather and interpret clinical and financial data.
  9. Ability to analyze clinical and environmental workflow systems and determine workflow assignments.

Preferred Qualifications

  1. Case management certification.
  2. Previous experience working in a collaborative care model.
